Anyone know how to include sample code or screenshots because the very basic form to submit one most definitely does not have the option to do so.

If you’re not seeing a place to add attachments, follow the advice in the second clause, namely “or in a reply once you receive a follow-up number from Developer Technical Support”.
Once you submit your request you’ll get an auto ACK. Reply to that with any extra info you want to supply, like your attachments. Make sure to include the Case-ID
in your reply.
And, yeah, we know that’s clunky. We’ve actively working to improve it.

I just received the status back as "credited". Which basically now means wasted because my 2 for the year has just reset too. I tried to use one before my annual renewal but the "clunky" process has just meant it's been wasted lol
